Angina pectoris: Patients with severe or advanced heart failure should receive doses of up to 800 mg three times a day for 10 days, starting on the 25th day of PAH therapy. Patients with left ventricular outflow contractility dysfunction, congestive heart failure, recent heart attack, or history of stroke or myocardial infarction should use riociguat at the lowest effective dose and not use it for treatment of heart failure.
Pulmonary embolism: There is a risk of venous thrombosis and at least two other serious complications in patients with pulmonary embolism.
